1.  Fee 50% of annual rate after 10/31 for applications for new owner or establishment.

2.  Penalty fee (% of food service license fee) for food service license renewal after April 30th.

3.  If the food service establishment operates less than 6 months/year the license fee will be ½ the regular fee as stated in the fee schedule.

4.  Schools, educational institutions and religious institutions are exempt from fees.  Food service operations held in conjunction with school, educational institutions or religious institution events but not under direct and exclusive control of the school, educational institution or religious institution are not exempt from fees.

5.  Fraternal, service, civic and other non-profit organizations shall pay one half the regular fee as stated in the fee schedule.  The organization shall furnish to the department evidence of its tax exempt status.

6.  Fees do not include any charges for water quality analysis.

7.  Fees do not include State of Michigan operation fees.
