Application procedures are as follows:

The supplier of water must complete the application form for the proposed water supply. A scaled drawing must be submitted indicating the location of the well and distribution system, the property dimensions of the site, all sources of potential contamination, isolation distance from the water supply, and all major structures and buildings using the supply.

Payment of the appropriate fee is also required. The Livingston County Health Department reviews the applications for accuracy and completeness. A well site evaluation is made, which involves an on-site inspection. The well construction permit is either approved or denied. Specifications for the well construction and identification of isolation distances are submitted.

After completion of the water supply and before the system is approved for use, a final inspection is made and two water samples are collected for analysis. Final approval for use is given when safe water quality is found and a water well record is received.
