For new construction wells:

The applicant completes the application form and provides a legal description, certificate of survey, scaled plot plan, or engineered plans. The applicant pays the appropriate fee at time of application. The Environmental Health Division will either approve or reject the permit, specify the isolation distances and who is to sample the water, and any other specified restrictions.

The well may be drilled by the homeowner or an approved licensed well driller. The well is subject to inspection at any stage of development. On site evaluation may be required by the Sanitarian before the well permit can be issued and prior to installation of the well. After the well is completed, before final approval for use can be given, safe water samples must be collected from the supply and a satisfactory water well record submitted. Any deed restrictions will also be noted on the permit.

For replacement wells:

The applicant must complete the application form and pay the appropriate fee, the same as for new construction. A site evaluation of the new well site and the old abandoned well may be performed before the permit is issued. The property owner is required to properly abandon the old well to prevent any potential ground water contamination. A water well record and safe water samples must be provided before final approval can be given to the replacement water supply.
